Since Liberty’s inception, we have been committed to providing supports for people who have intellectual and developmental disabilities.
Our expertise includes operating large and small intermediate care facilities, safely transitioning people from institutional settings to community-based living arrangements, right-sizing and closing state-operated facilities, and hiring clinicians and professionals who have experience supporting people with intellectual, developmental and physical disabilities.

- Liberty has fully privatized and operated large state-owned intermediate care facilities and revitalized service systems to improve quality supports for the people who live there, as well as their families and the staff who support them. In some cases, we continued to operate those facilities for at least ten years; in others, at the state’s request, we have downsized and closed the facility, carefully planning and safely placing people in community settings with individualized supports, significantly under budget and ahead of the state’s timetable.
- Liberty has provided administrative and clinical management teams, often within days or weeks of an urgent request, to assume full responsibility for operation and management of large state-owned intermediate care facilities in order to promptly resolve issues raised by the United States Department of Justice (USDOJ) or the Centers for Medicaid and Medicare Services (CMS). The Liberty management team has worked with state staff at facilities all across the country to successfully restore CMS certification and restore federal funding.
- Liberty has worked with community waiver service providers to develop clinical systems and train provider staff in methods to support individuals with intensive needs for medical, nursing, physical therapy, occupational therapy, speech therapy and/or behavior support. Liberty has developed sophisticated methods for expert on-site consultations and no-cost training DVDs for direct care personnel for safely managing the most common causes of mortality in the IDD population, referred to as the “Fatal Four,” as well as other high-risk health and behavioral issues.
- Liberty has provided expert clinical, management and technical consultation services to state-operated intermediate care facilities to develop plans of correction and provide recommendations for incident management and system and service improvements needed to comply with consent decrees, CMS deficiencies, and USDOJ suits. Liberty develops a detailed plan of remediation in partnership with facility management to resolve critical issues and enhance quality services.
- Liberty has privatized state quality improvement system oversight of HCBS waiver provider services and supports in accordance with the CMS Quality Framework, in collaboration with the Council on Quality and Leadership (CQL). This has encompassed the development and implementation of new survey tools, incident review systems and mortality review processes for an entire state system, including a new web-based database to analyze the results.
